Make every miss change the next set

Missed-question review is not a punishment list. It is a short loop for turning an uncertain decision into a repeatable one. Read the explanation, name the exact gap, then choose the smallest next rep that can prove the repair.

01 · Separate the signal

Miss, guess, and slow are different problems

A wrong answer can mean the rule was missing, the facts were misread, or the answer was selected with low confidence. A correct answer can still need review when it was a guess or took too long. Use the saved explanation, confidence, and pace signals together.

02 · Name the repair

Write one sentence before adding volume

Describe the controlling rule, the tempting distractor, or the calculation step that broke. One precise sentence is more useful than rereading an entire chapter without knowing what should change.

03 · Prove the transfer

Retest the skill in a new shape

After the explanation, return to a same-topic question, a cumulative set, or a Lite TBS. If the skill survives a changed fact pattern, keep it active; if not, let RECON bring it back again.
